The Service Provider agrees to provide nutrition services to the Client, including meal plans, nutrition coaching, and accountability support as outlined in the chosen package. Services are personalized based on information provided by the Client.
The Client agrees to:
Provide accurate and complete health, dietary, and lifestyle information. Communicate any specific dietary restrictions, allergies, dislikes, or other preferences upfront.
Adjustments will not be made after meals are created based on provided information.
Follow the recommended plans and guidelines provided by the Service Provider to the best of their ability.
Full payment for services is required upon signing this contract unless otherwise specified.
Payment is non-refundable once services commence, except under specific circumstances outlined in the Service Provider's refund policy.
The Service Provider reserves the right to adjust service fees with prior notice.
The Client may reschedule coaching sessions with a minimum of 24-hour notice. Cancellations or rescheduling with less than 24 hours’ notice may result in the loss of that session.
In the case of an unforeseen emergency, the Service Provider will work with the Client to reschedule sessions at a mutually convenient time.
The Service Provider is a certified nutrition coach but not a licensed medical professional. The information provided is not intended as a substitute for medical advice, diagnosis, or treatment.
The Client is encouraged to consult with a qualified healthcare provider before making significant changes to their diet or lifestyle.
SEAK Nutrition Solutions and Sandra Harpe are not liable for any adverse outcomes related to the implementation of provided meal plans or recommendations.
This contract shall be governed by and construed in accordance with the laws of the state of California.
By signing below, both parties agree to the terms and conditions of this contract.
